Background Screening Requirements

The USA Hockey Background Screening policy is described in the SafeSport Program Handbook (Section III Screening and Background Check Program). USA Hockey policy requires each Affiliate to establish and maintain a Background Screening Program for Coaches, Officials, and Volunteers. With the exception of Florida Officials who are required to screen annually, an approved screen shall be valid for two (2) years. All previously screened persons must be re-screened every two (2) years. You are invited to review the Notice to Users which discusses important privacy and consumer information about the background screening process.

In the Southeastern District, background screening for Officials is administered at the District level. The Southeastern District Screening Coordinator, Jacqueline Zee Howard, maintains a database of screening records for all Southeastern District Officiating Program Officials and advises Southeastern District game assigners as to a Referee's background screening status. Officials are not authorized to work games if they are over 18 years old (all ages in Florida) and do not have a current background screening report on file.

How to Request a Background Screen

With the exception of Florida Officials, who are required to use the SAHOF background screening agent (scroll down for further detail), all Southeastern District Officials should use USA Hockey's national background investigation organization, the National Center for Safety Initiatives (NCSI).

Please follow the directions below to start the online registration process.

Go to NCSI, click on “START YOUR BACKGROUND SCREENING NOW”. (Please note that this link will be shut down between May 1 and August 1 to correspond with the beginning of the regular ice hockey season, which begins August 1st).

STEP 1: Enter Self-Registration Number: 53120419
STEP 2: Enter your information as requested
STEP 3: Pay the required fee
STEP 4: Provide Legal Authorization and Certification

It is important when entering your name, that you use your full legal name as written on your current ID (state driver's license, passport or state ID with name and DOB). Please also verify that your social security number is typed correctly. Using nicknames or failing to type a correct social security number may delay or invalidate your background check.

Once you have submitted your information you will receive a confirmation page with a 16-digit Applicant ID Number. The background check generally takes 10 business days to complete and you may check your status on the NCSI website under the “CHECK YOUR BACKGROUND SCREENING STATUS” link at www.ncsisafe.com. The fee for a NCSI Background Screening is currently (2018) $21.25 for most Officials. Many might see additional fees due to various county, state or international checks.

If you are a Florida Official, regardless of age, you must complete an annual (not biennial) background screening using the SAHOF screening agent, Blueline Services. This is required by Florida law and is not the result of any new requirement from USA Hockey or its affiliates. Florida Officials should go to the SAHOF Background Screening Registration page and apply online for a background screening. The web site is for Florida background screening for Coaches, Officials, and Volunteers. Be sure to use your USA Hockey Referee registration confirmation code (not your Referee number) and apply for a background screening as a Referee.

If you are both a Coach and a Referee, you should request a background screening as a Coach, following your Affiliate rules and forward a copy of your Coach background screening results to the Southeastern District Screening Coordinator, to get credit for your background screening in the Officiating Program. A Coach's background screening is not automatically transferred to the Southeastern District registration database.
